How they differ

1. “Appointment” is the trap. A doctor’s or hair appointment is a booking: 병원 예약이 있어요 ○ / 병원 약속이 있어요 ✕. Meeting a friend is an arrangement between people: 친구랑 약속이 있어요 (I have plans with a friend) ○ / 친구랑 예약이 있어요 ✕.

2. 약속 is also a promise. 다시는 안 늦겠다고 약속했어요 (I promised I’d never be late again). 예약 never means a promise. Useful verbs: 약속을 지키다 (keep a promise), 약속을 어기다 (break it), 약속을 잡다 (set up plans).

3. 예약 takes the place, seat or service as its object. 식당을 예약하다 (book a restaurant), 표를 예약하다 (book a ticket), 예약을 취소하다 (cancel a booking). You can’t 약속하다 a restaurant: 식당을 약속했어요 ✕.
