When to use it
Attach -고서는 to a verb stem — no vowel rule, ㄹ stays, no tense in front. It is -고서 with the topic particle 는 added, and that 는 is what makes it contrastive.
The second clause is negative: 그렇게 하고서는 좋은 결과를 기대할 수 없다. Having done that, the good outcome is off the table.
Very often the first clause is a negative too, giving “without doing X, you cannot Y”: 준비하지 않고서는 통과할 수 없다. That is the commonest shape by far.
Keep it apart from -고서야, which says the second thing only becomes possible after the first. The two are near-opposites.

-고서 simply sequences two actions. Adding 는 turns it into a contrast and calls for a negative conclusion.
밥을 먹고서 나갔다. (ate, then left) / 그렇게 하고서는 안 된다. (do that and it won’t work)
-지 않고는
The shorter form of the same idea, and equally common. -지 않고서는 is a shade more emphatic.
