When to use it
Attach -기에 이르다 to a verb stem — no vowel rule, ㄹ stays, no tense in front. 이르다 is a 러-irregular verb: 이르러, 이르렀다.
It is almost always used in the past, since it reports the culmination of something already run its course: 결국 폐업하기에 이르렀다.
There must be a process behind it — a build-up of causes. A sudden one-off event does not suit the phrase.
The outcome is often grave, though not necessarily: it can equally mark the point at which something was finally achieved.

The phrase needs a build-up behind it. A sudden mishap takes -고 말다.
